Camp Medical Forms

All camp medical forms must be completed online in the US Customer Portal or Canada Customer Portal for 2025 camps.

For camps in the US, visit the NBC US Customer Portal-follow the steps to log in to your account, pay your balance, complete required camp medical forms and view camp information: https://secure.nbccamps.com/Login

For camps in Canada, visit the Canada Customer Portal- follow the steps to log in to your account, pay your balance, complete required camp medical forms and view camp information: https://secure.sportscampscanada.com/login

A doctor's signature is not required to complete these medical forms, nor is a camper physical. Campers without a completed Medical Release Waiver will not be allowed to participate at camp.

How to Log In or Create a New Account:

US Camps: Go to https://secure.nbccamps.com/Login

Canada Camps: Go to https://secure.sportscampscanada.com/login

• Enter your email address.
• If you are a returning participant with an existing customer portal account, you will be directed to enter your password and log in to your account. If you do not remember your login information, please click "Forgot My Password" and follow the prompt.
• If you are a new participant, you will be prompted to create an account. Enter your household/billing account information and create a password. Click "Create Account".

How to Add Camper(s) to Your Account:

• In the MY CAMPERS tab, click the "Add Camper" button on the top right.
• Enter your camper's Confirmation # and Date of Birth in MM/DD/YYYY format and click Find Campers.
• If you do not know your camper's confirmation number, please click on the Forgot Confirmation #? link, or call the NBC Camps office at 800-406-3926 or email nbc@nbccamps.com.
• Once your camper is successfully added, go to MY ENROLLMENTS and you will see their registration.

How to Complete Your Health & Release Forms:

• On the MY ENROLLMENTS dashboard, you will see your camper's enrollment and a red notice "2025 NBC Forms Incomplete".
• Click the 'Start' button and review the pop-up message in full, then click 'OK.' You can now complete the forms.
• Please allow several hours for the forms to populate in the Customer Portal. Then you will be able to "View" or "Edit/Delete" the forms.
